OUDS Welfare Feedback Form

This form is your opportunity to anonymously and confidentially report any welfare issues you have had as part of your membership of OUDS - this can be regarding the Society as a whole, specific experiences on productions, or anything else you wish to flag.

You have the option to submit your concern to the following people:

• OUDS Welfare Officer

• OUDS President

• University Drama Officer

There are 3 forms below, and each form will send its responses to the corresponding individual listed above. If you would like more than one of the three individuals listed above or anyone else on the committee to receive your submission, please submit only one form and list the additional recipient(s) in the last question.

Information submitted will not be shared with any other individuals unless you wish to do so. Concerns which need addressing with other Committee members will be handled anonymously unless there is a risk to a group's or individual's safety, at which point an issue will be escalated to University welfare services.

There is a question at the bottom regarding further action - if you want further action to be taken, please do leave an email address. This isn't mandatory, and you are more than welcome to remain anonymous - if you do leave an email address, you will receive an email from whoever you reached out to following up on your feedback.
